The initial paperwork, which you will need to complete prior to scheduling, includes requests for insurance information, office policy review and signing, and completing brief checklists about what you have been feeling that led you to begin therapy.  You can expedite this process by downloading them from the link below and either bringing the forms to the office or by faxing a copy and bringing the originals to your first appointment.  If you choose to fax them they should go to (770) 435-9357.
